Grant Evaluation Report: Sharing Success and Learning Lessons

We hope our grant funding enabled many seafarers and their families to receive help and benefit from the services and support provided by your organisation. 

We are interested to learn what difference our grant funding made to the work of your organisation and the lives of the seafarers you supported.

We now require you to complete a Monitoring & Evaluation Report for grants awarded in 2019.  Please complete a separate report for each grant award.   

Please use this Survey Monkey report to provide as much detail as you can about how you used your grant award. This is required in respect of the Terms & Conditions which were provided with your Grant Offer Letter.
