1. Community Care of patients in Residential Care Homes

The purpose of this survey is to gather data to show the level of input within Residential Care Home settings provided by teams from Derbyshire Community Health Services NHS FT.  The information gathered will be analysed and the findings used to inform recommendations for current and future development opportunities of care support workers within the Residential Care Home settings, including the implementation of new roles, specifically the Nursing Associate (equivalent to Band 4).  The data will be kept strictly confidential.  You will only be contacted if you have expressed a wish to be involved.
