You are receiving this survey because you have either:

1. Taken part in, or supported, an ArtReach festival or event between April 2021 and April 2022 as an artist, workshop leader, performer, freelance evaluator, marketing support, volunteer or speaker.

2. Submitted an application for a recent job or project vacancy

As part of ArtReach's monitoring activities in line with our Equal Opportunities Policy, we would like to invite you to complete a short questionnaire so we can understand the diversity of the people with which ArtReach works, engages and supports.

This data is used internally and to report to our core funders e.g. the Arts Council England.

Your response is really important to us and so we have designed the form to be quick and simple. It should take less than 3 minutes to complete.

Responses will be handled confidentially by the ArtReach team and reported to external partners anonymously.
