This survey is for:
-parents and carers of children and young people with Special Educational Needs and/or Disabilities (SEND), who live in Somerset or use Somerset services.
- Young people
-Practitioners working with 16-17 year olds in Somerset

The following survey will be used to inform practice in Somerset, with the view to improving things for all families and recognise services families’ value. The information collected will be shared anonymously with the SEND board and team in Somerset County Council and Somerset Clinical Commissioning group, who are developing the pathway for ADHD and Autism. No personal information will be shared without your consent to do so.

We estimate the survey will take approximately 10- 15 minutes to complete. All data from this survey will be stored securely inline with the forums GDPR policy which is available on our website.
