Introduction

This Alopecia UK Service Directory application form is for businesses, operating in the UK, that provide a service that is relevant to those with alopecia.
 
The Service Directory is currently suitable for businesses providing the following services:
  • Wigs
  • Toppers
  • Hair systems
  • Extensions
  • Headwear
  • Permanent make up (PMU)
  • Scalp micropigmentation
  • 'Alternative hair' care products
  • Wig styling services/alterations
  • Wig bank services *
  • Lash & brow products

*Wig Bank services can be added as a tag free of charge. Any businesses with at least 50% of donations going towards Alopecia UK may also take part in Wig Bank Wednesday in our closed FB group. (This will be explained further in your welcome pack).

Pricing
All prices can be found in our advertising pack which you can view on our website here. 
All businesses who are renewing their listing with us will be given a 20% loyalty discount on their invoice total. 
Businesses who apply after the 31st March and therefore have 'part year' subscription dates will be given pro-rata pricing.

Please note: Service Directory applications will be processed quarterly throughout the year as stated above.  This means you will not hear from us until this time. For example, if you complete your application on the 1st April, you may not hear from us until the end of June.
Application Process

Step 1: Complete this form.

This form will take you through the application step-by-step. You will be able to click forwards a page by using 'Next' or go back a page by clicking 'Previous'. You will be able to do this until you click 'Done', on the final page, which will submit your application to us.

This application has six sections. Please allow yourself 20 - 30 minutes to read through and complete this application.

To complete this form you will need:
- 300 words describing your company/business/service
- A banner photo (landscape, ideally 1600px by 700px at screen resolution of 72dpi).
- 3 photos for body of webpage if desired (this is included in your basic package)

You may wish to prepare the above before you begin.

Step 2: Alopecia UK will review applications on a quarterly basis (dates above). Upon reviewing applications you will receive an acknowledgment email. You will also receive an estimate. Your estimate will outline the total price based on your choices in your application. If you would like to add/remove any options it will be at this time. 

Step 3: Once you have accepted your estimate you will receive your invoice. Your invoice will include payment terms and our preferred payment method. 

Step 4: Once your payment has cleared in the Alopecia UK bank account your listing will be created and added to the Service Directory. You will then receive a confirmation email with our welcome pack and any additional information.
