This survey asks questions about support that you may have received from bereavement services in the either the Bristol, North Somerset or South Gloucestershire areas.
IMHN and CASS are looking to hear from people who have accessed bereavement support; and bereavement support following a death by suicide; so that we can support work to improve local support available in the area.
Anonymous responses will be shared with the other organisations that are support this work. Responses you submit will be kept safe in a way that is recommended by the General Data Protection Regulations (GDPR).
