Dementia Carers' Survey

Wirral University Teaching Hospital recognises that carers and relatives play a vital role in the care of patients with dementia. We are undertaking this survey to gain feedback about your experience of the support that was provided to you at Wirral University Teaching Hospital.

Your responses will be treated confidentially and will be used to improve the service we provide.
