Your thoughts and ideas are valuable and will help shape how condom distribution services are delivered in Derbyshire. All answers are confidential. We will anonymise how we use the survey results so that no one will know who has replied.
At the end of the survey you will be invited to take part in a competition to win a one of two £250 Love2Shop gift cards, which can be spent at over 90 retailers in store or online: • there will be one draw and two winners will be selected . it will take place on 21st February 2020. • you will be asked to leave a contact mobile phone number so we can contact you if you are a winner.
Entering the competition is optional and any contact information you provide will be separated from your responses to the survey questions so that your answers are anonymous.
The questionnaire is for 13-24 year olds only, who live in Derbyshire and Derby City Residents.
We estimate the survey will take about 7 minutes to complete. Most questions are multiple choice.
