Education, Health and Care Plan Process Survey 

General Information

This survey has been co-produced by Essex County Council and Essex Family Forum and is being conducted to find out what is working well within the Education Health and Care Plan (EHCP) process (including Annual Reviews) in Essex and where the experience for families needs to be improved.  The survey will take less than 10 minutes to complete.
1.CONSENT - By providing consent, you are agreeing that personal information about you will be collected anonymously stored securely and will not identify anyone personally. The anonymised data will be used and shared with relevant organisations for the purpose of improving SEND provision within Essex and England but will not identify anyone personally.

Once consent has been provided you have the right to withdraw your views and your views will be removed.  However where data has been anonymised and amalgamated in certain circumstances it may not be able to be excluded.
