Questionnaire

We know that the effects of Covid over the last two years have been difficult  for many people. We want to improve the support people get in the future.

To do this we are working with the Public Health team, Cornwall Council as they are working to help make services better. To help get services right, we are speaking to people across Cornwall and Isles of Scilly? about how the pandemic has affected their lives.

We would like to better understand the challenges you’ve faced as well as what has gone well for you during the pandemic. We would also like to know your ideas for improving the support you get. There are a number of things we would like to ask about including your wellbeing. Everything you share will be anonymous and won’t be shared with anyone else.

If you would like to take part, please complete the attached consent form. This gives your consent to take part in this survey and to share summary findings with Public Health to inform future services. If you’re unable to complete it and have a carer, with your permission they can complete it for you.

Thank you for taking the time to complete this survey, it should only take 10-15 minutes to answer. To help improve future services we will share the results with Public Health Cornwall.
