This survey is designed give an insight to your vet as to whether your cat could potentially be hyperthyroid - if they have reason to believe your cat may be hyperthyroid or if there is any other concern raised as a result of the answers given, a member of the team will contact you.
In the meantime, if you have any worries or concerns for your cat's health please contact your vet immediately.
At the end of this survey we will ask for your contact details so that a member the vet practice's team can contact you regarding your results.
