Create London - Recruitment equality and diversity monitoring form 

As a publicly-funded arts organisation, Create London is required to statistically analyse the demographics of our staff, trustees, freelancers, artists and applicants. The information we ask for includes gender, sexual orientation, age, ethnicity and disability.

The information collected via this form is anonymous and used solely for the purposes of monitoring and evaluation.  It does not form part of the shortlisting process. Forms are kept for a maximum of six months and then deleted.
