Fifth Sense Touch and Smell Cards

We'd be grateful if you would take the time to complete the following survey to give us your feedback on using the Fifth Sense Touch and Smell cards to do a simple smell test with your patients.  
You only need to complete questions 1-10 once, but please complete questions 11-14 for each patient you see who uses the Touch and Smell card.

This is a service evaluation survey to capture data on whether a simple smell test is helpful to you when seeing patients reporting problems with their sense of smell. Data from this survey will be used by Fifth Sense to report to our funders and may also be used to inform a research paper in relation to the Touch and Smell cards.  All data used in this way will be completely anonymised.
