Your school wants to make sure they are looking after everyone the best that they can and want to know how you are feeling and what more they can do to support students.

We will not ask you your name so no one will know what you have written but if you would like to talk to someone about your answers please talk to someone in your family, your class teacher or someone you trust in school.
We want to support all schools in Sheffield so we will be sharing this information with our evaluation team but they will not know your name.
