Electronic Health Check
23rd January 2022
Cardiff Met
Non Thomas NY Open

Event requirements:
ALL spectators, coaches, athletes, officials & coaches must show evidence of a negative lateral flow test from the morning of the event via the NHS to gain access to the building. The only exceptions for this will be anyone aged under 11.  Face masks are compulsory at all times whilst in the venue - the only exceptions for this are whilst eating/drinking, and for athletes whilst either warming up or competing.

Additional Welsh Government advice:
Welsh government has said it is “strongly advising” that people “flow before they go”. This means taking a lateral flow test before going out – for example to a Christmas party, shopping, visiting friends or family, to any crowded or busy place, or before travelling. If the test is positive, they should not go out but arrange for a PCR test and self-isolate, the government said.

To complete the process this **Electronic Health Check** must be complete.
**This should only be completed before leaving to attend or when arriving at the venue- as this means YOU WERE IN THE VENUE.**

5.I confirm I don't currently have any symptoms of coronavirus (COVID-19) Also you agree that you have followed the government guidelines in respect to international travel & contact with other possible cases- all information:  https://www.gov.uk/coronavirus

The main symptoms of coronavirus (COVID-19) are: a high temperature – this means you feel hot to touch on your chest or back (you do not need to measure your temperature) a new, continuous cough – this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual) a loss or change to your sense of smell or taste – this means you've noticed you cannot smell or taste anything, or things smell or taste different to normal

What to do if you have symptoms
If you have any of the main symptoms of COVID-19, even if they're mild: Get a PCR test (test that is sent to a lab) to check if you have COVID-19 as soon as possible.

Stay at home and do not have visitors (self-isolate) until you get your test result – only leave your home to have a test. Check if people you live with need to self-isolate.
