Share you views on how you would like to collect your hospital prescription

The coronavirus pandemic has resulted in lots of changes in the hospital and it’s allowed us to test new ways that people can get their hospital prescriptions. We could make some of these changes more permanent, but we need know what you think about these. Please tell us your views by answering the questions below.
 Your answers are completely anonymous and the questionnaire will take around 5 to minutes complete.  The survey closes on Friday 5th February.
