3.Have you been awarded PIP mobility component, or a higher rate of the mobility component, after August 2018 even though your condition has not changed?  For example, because you had a planned review of your award?(Required.)
4.On approximately what date did you get a decision letter telling you that your case had been looked at under the LEAP process, but there had been no change in the decision?(Required.)

6.Did the DWP contact you without prompting about this?(Required.)
7.Have you challenged the decision? This would initially be by a mandatory reconsideration and then an appeal.(Required.)
