Carers Survey
This survey has been devised by the Carers Champions & Social Prescribing Co-ordinator in the GP surgeries within the Rural Practice Network – Bruton, Wincanton, Castle Cary, Queen Camel and Milborne Port - who have identified a gap for support for unpaid carers aged 18 to 60. We would like to hear from carers of this age group about their thoughts on the kind of support and connection with other carers that they would like.
Obviously at the moment, due to Covid - 19, we are living with a number of restrictions and social distancing measures are still in place, but we are still keen to receive your thoughts, comments and ideas to help us plan ahead as well as decide how to support you now.
The survey would like to find out what kind of information, advice and support you feel you need and whether you would like to be able to take respite breaks with other carers and perhaps meet up regularly. Your ideas would be warmly welcomed! It should only take 10-15 minutes of your time.
There is a text box at the end for you to freely write your comments and thoughts on the issues affecting you and how we can address this together. The Carers Champions will then look at any common threads and try to find a way through that is supportive and helpful to you.
If you are a carer aged 18-60 and NOT living in the catchment area of these 5 GP surgeries, you are still welcome to complete the survey and Spark Somerset can pass on your survey results and comments to the Carers Champion in the GP surgery where you live
Thank you so much for taking the time to complete this survey. We will be in touch in due course.
