Improving the Physical Health of People with Severe Mental Illness

It is known that the life expectancy for people living with severe mental illness, eg schizophrenia, Bipolar Disorder or Psychosis, is significantly shorter than average. We are keen to understand what difficulties and barriers people experience in trying to stay physically well. What helps you and what hinders you?  We will present the findings of this survey to those who can help improve future services.  We would very much appreciate your support by telling us your own experience. You do not need to give us any personal data (eg name, address). Any information you provide will be kept entirely anonymous.

If you have any questions regarding this survey or would like to know more, please contact Aileen Power by email: aileen.power@healthwatchderbyshire.co.uk or phone: 01773 880786 (office),  07704 005303 (mobile).

If you are a carer completing this form on behalf of the person you care for, please understand ‘you’ and ‘your’ to mean the person that you care for.
