HUL Virtual Consultation Service Family and Patient Survey

The Hand and Upper Limb (HUL) team along with its supporting Occupational Therapy (OT) team would like to find out a bit more from our patients about how the online consultations are meeting your needs.  Can you please take 5 minutes to complete this survey?
