NHS Newcastle Gateshead Clinical Commissioning Group would like to understand your views on a change in the way care for people with Type 2 Diabetes is organised in Newcastle. This change will affect people whose condition is stable. The aim is for diabetes care to be delivered closer to home with reviews taking place at your GP practice. The team from Newcastle Diabetes Centre will support GP practices across Newcastle to provide this care for people with Type 2 Diabetes.
