TCST Service Improvement Tools for Cancer Rehabilitation: Evaluation Survey

Introduction


The Service Improvement tools for Cancer Rehabilitation were developed by the Transforming Cancer Services Team for London (part of Healthy London partnership), as part of a project funded by Macmillan Cancer Support. Both tools have been developed through extensive stakeholder engagement with the service user voice at the centre. The tools are designed to be used alongside each other and aim to outline a clear framework for what good cancer rehabilitation looks like.

We would very much welcome your feedback on both tools and would be grateful if you could complete this survey for each tool after each use. All feedback will be used to make improvements and refinements and this survey should take no more than ten minutes to complete. This survey is also available as a paper version here
