LRI Adult ITU therapy dog: staff questionnaire

This questionnaire should be completed if you are a member of staff and  have interacted with a visiting therapy dog either with or without patient involvement.
For clarity, an Animal-Assisted Intervention (AAI) is where the dog has been with a patient during a medical, nursing or therapeutic intervention.
