Children and Young people with Disabilities - Parent/Carer Questionnaire

Welcome to My Survey

Redcar and Cleveland Borough Council would like to hear from parents and carers of disabled children and young people aged 0-25 years. This questionnaire will capture your feedback so we know what is working well and areas for improvement. The questionnaire will cover 3 areas:

Our self-referral Short Breaks Clubs offer
Universal services/activities available in the Borough for children and young people with disabilities
Carer Support

This survey will take about 20 minutes to complete and will close on 11 April 2021.

If you have more than one child who has a disability there will be an option to provide a response for each child.
