Thank you for taking the time to give us your feedback. It is extremely valuable to the hospital and will help us to look at areas that we are currently doing well in, and where we need to focus on in the future. 

This survey is only to be completed  by Registered Nurses & Midwives

The wording of the questions has been taken from an American Self-Assessment. Where possible, we have provided examples of what the question may refer to, to make it clearer what the question is asking.
