On behalf of NHS Health Boards, we are collecting suggestions for amendments to the Approved List from our members and their pharmacy teams, which can be for additions to the list, replacements or deletions.
This feedback will be collated and passed on to the group that reviews the Approved List at its 3-yearly review, who will assess each suggestion from the network in the same way that formulary suggestions from NHS Board colleagues are assessed. The outcome of this review will be published towards the end of the year.
Please take care to fill out each section fully. Attach or include a link to any supporting evidence for your suggestion and (particularly if this does not exist), clearly explain your rationale for making the suggestion. By submitting a suggestion, you agree to your information being passed on to the review group, who will only contact you if they require any further detail or clarification.
Please note that you will not receive direct feedback on your suggestion, but that each new version of the Approved List will include a brief summary of the most common requests and the outcome of decisions related to these.
