We are asking parents and carers about the health therapy services that their children have used at Kaleidoscope.

These are physiotherapy, speech and language and occupational therapy.

We have been asked to feedback on what parents/carers think about the services that have been received. This will help them to make improvements and to also understand what is working well for families.

The deadline for completingt his survey is 14th June.
