1. Introductory information

Please accept our condolences for the recent loss of your loved one. We appreciate that this is an extremely difficult time and you may wish to look at this survey at a later stage.

Across the Harrogate and Rural District area we are constantly trying to improve the quality of care we deliver and we use feedback from patients and their families to help us do this. One area that is very important to us is the quality of the care of dying patients, and the care given to their families and friends. To find out more about this, we ask for the views of people who have been close to someone who has died. We ask about the care the patient received in the last days of life, in whatever setting they were cared for (hospital, home, care home or hospice), and the support their family and friends received.

We realise this survey may bring strong memories and emotions, and that looking at it for the first time may be difficult. There is absolutely no obligation to complete the survey; it is entirely voluntary, or you can ask someone else who may be better placed to complete it if you prefer.

All information entered is sent to Harrogate and District NHS Foundation Trust, the organisation running the survey, and will be strictly confidential. No one will be identifiable in the report produced. In accordance with General Data Protection Regulation guidelines this report will be shared with healthcare teams who look after and plan care for dying patients and their families throughout Harrogate and Rural District.

Should you have any questions or concerns about the care you or your family member or friend received and would like to speak to someone, please contact a member of the organisation or healthcare team which looked after them, or call the Harrogate District Hospital Patient Experience Team on 01423 555499 and they will be able to direct your call appropriately.

If you would like support with your bereavement, please contact Just B on 01423 856790. This is a local charity providing bereavement support to people within the Harrogate and Rural District community. They can also provide information about other support if you live outside this area.
