Thank you for taking time to complete this short questionnaire. it should only take a few minutes and can help us make sure that we continue to research and identify how we can best support anyone who has been diagnosed with cancer. Your support of this survey will help us ensure that we are providing the support that is needed. 
1.Have you or a close relative been diagnosed with cancer in the last 3 years?
2.Have you / they been offered or accessed any of the following support?
3.Did you / Do you have a cancer key worker? ( a key worker is a named specialist that you can contact  at any time with queries regarding your diagnosis, treatment etc)
4.Are there any services we could provide that could support you or your family?
5.Is there any support that you really need / needed that you struggled to access?
6.What is the main barrier for you accessing any kind of support?
7.Would you be interested in any family / friends group support options such as memory making opportunities?
8.Is there anything that you would like to see Cancer Aid offer, that we do not currently provide that could extend or change what we currently offer?
