This survey is for parents and carers of children and young people aged 0-25 years with Special Educational Needs and/or Disabilities (SEND), who live in Somerset or use Somerset services. 

In March 2020, Ofsted and the Care Quality Commission (CQC), conducted a joint inspection of the local area of Somerset, to judge the effectiveness of the area in implementing the disability and special educational needs reforms, as set out in the Children and Families Act 2014. The inspectors found significant weaknesses and the local area and a plan of how the local area would address this was created.   

The following survey will be used to inform Somerset Parent Carer Forums report as part of the monitoring of the Written Statement of Action. The information collected will be shared anonymously with the  representatives from the Department of Education and NHS England, the SEND Improvement board, Somerset County Council and Somerset Clinical Commissioning group. No personal information will be shared without your consent to do so. 

 
We estimate the survey will take approximately 10- 15 minutes to complete.
