The Inclusive Involvement Community

Introduction

As we grow, we want to ensure we are inclusive of people from all communities. To be able to do this, we need to know more about the profile of the people who sign up to our inclusive involvement community.

Please can you help us by completing an anonymous questionnaire? You can skip questions or state you would prefer not to say. It will take a couple of minutes.
Completing this anonymous questionnaire helps us to know who may be underrepresented among our subscribers and community. This questionnaire is voluntary and all data is collected anonymously and held in line with the General Data Protection Regulations (GDPR). Thank you.

If you have any questions about the form you can contact us via email at hello@shapingourlives.org.uk or by telephone at 0345 241 0383.
