Introduction

Cardiac rehabilitation is a programme of exercise, education, and psychological support. It has been shown to:
  • Reduce hospital admissions
  • Reduce the number of cardiovascular disease related deaths
  • Reduce the risk of further cardiovascular events
  • Improve quality of life
Nationally, only half of all patients eligible for this service access it. There are many reasons for this, and NHS Birmingham and Solihull, along with other NHS partners are keen to understand why.  We intend to work with our partners to act on feedback, and support patients to access and complete these programmes in a way that is most suitable for them.
 
You can help us do this by completing this short questionnaire, your answers will remain anonymous however the outcome of this questionnaire will be shared on the Healthy Hearts website: www.bsolhealthyhearts.nhs.uk

How we use your information
The information you provide will be held by NHS Birmingham and Solihull, to help us redesign the Cardiac Rehabilitation Programme across Birmingham and Solihull and identify barriers that prevent patients attending the programme.
 
NHS Birmingham and Solihull will not share your personal information with third parties. However, we will publish the results of the survey in a way that does not identify you.
 
NHS Birmingham and Solihull will retain this information for six months after the service redesign has been agreed and then delete any information that identifies you.
