Salisbury Surgery Patient Engagement Survey

NHS Hammersmith and Fulham CCG has been advised by Dr Muthiah, that he wishes to retire and therefore hand back the practice contract he holds with NHS England with effect from 30 January 2021. This means Salisbury Surgery practice will close on 30 January 2021. The current premises will not be available for the provision of GP services after that date.

The Primary Care Commissioning Committee of the CCG will need to make a decision at its meeting in December on the way forward. It is likely to be asked to consider closing the practice and supporting patients to register with a different GP locally.  However, before this recommendation is put to the Committee the CCG wants to  take into consideration any feedback from patients.

Please share your views by filling out this short questionnaire. It is for Salisbury Surgery patients only and must be completed by  1pm on Friday 18 December.
