Public Events Registration Form

Come and tell us your thoughts on some ideas of how health and care services could work better together for local people.

These events will be informal and relaxed and there will be an opportunity to look at a number of information posters and give us your views.

Arrival is from 6.00 and the event will start at 6.30pm. Tea/coffee and light refreshments (biscuits/fruit) will be available.
