Winter Pressures 2022 Panel Member Expression of Interest form

The Bradford District and Craven Health and Care Partnership has offered the local VCSE £1.2 million over 12 months to spend on developing 3 lead Wellbeing Hubs, and 3 Mini-hubs, across the district. This funding comes from NHS E/I (England/Improvement), and is specifically targeted at reducing Winter Pressures on Acute Care Services.

The location of these Hubs has been decided based on Urgent Care, First Response and Mental Health data, and will be based in:

- Keighley – Lead Hub
- BTHFT – Lead Hub
- Bradford East – Lead Hub
- Manningham – Mini-Hub
- Shipley – Mini-Hub
- Tong – Mini-Hub

The hubs must do the following:

- Provide a wide open door for people in those localities to access services which meet their immediate health and social needs.
- Provide evidence-based interventions, delivered by existing commissioned providers such as Mental Health, Alcohol, Domestic Violence, Carers’ Support, Physical Health and Welfare Benefits Advice.
- Work closely within the system to develop a seamless referral system from all health and care providers and a robust monitoring and evaluation system from the beginning.
- Provide staff to join the dots across all community services including those provided by the Local Authority, Health, the Mental Health Transformation work and the local Primary Care Networks (PCNs).
- Link closely with the Acute Trusts, Multiple Agency Support Team (MAST), and the Re-enablement Co-ordinators, to provide support for those on discharge pathways from the hospitals, and those who can be signposted safely from the Emergency Departments.

This funding will allow us to develop two proof of concepts:

- A community embedded approach
- A diversion-focused approach from Acute Care which will allow quick redirections of appropriate people from the Emergency Departments straight into a VCSE co-ordinated service, which should be able to meet their social and non-clinical needs. This will allow specific links to be developed with Core 24, Mental Health Services and MAST.

In order to maximise the spread of investment across the VCSE Sector, and create clear boundaries within the partnership, the proposal has been split into 7 Lots.

It is vital that these services will need to be implemented as quickly as possible, and at the latest by 31st March 2022.
