Patient Experience Survey

Healthwatch Blackpool would like to hear about your recent experiences of using health and social care services. The following survey has been designed to gather feedback and opinions of these recent experiences whether positive or negative. Your feedback will be used by Healthwatch Blackpool for the purpose of improving these services. Giving feedback only takes minutes, but could bring long term improvements to your local services.

Before you start it’s important to know:

· Your views and options will be treated confidentially and anonymously.

· We don’t handle individual complaints BUT can signpost you in the right direction for complaints and information regarding health and social care services.

· We look for trends within your feedback and produce summary reports of all views received showing the biggest issues raised, to help us decide which areas to prioritise for further research.

· All information will be handled and shared in accordance with General Data Protection Regulation (GDPR) and Data Protection Act 2018. We will take all necessary steps to ensure that your information is treated securely.
