In order to gain feedback and help the Trust improve its responses to requests for information under the Freedom of Information Act 2000 (FOI) and Environmental Information Regulations 2004 (EIR), we would be grateful if you would complete this short survey about our service.
The information we collect will be used to monitor and improve the quality of our responses to such requests for information and will assist the Trust in reviewing and improving procedures and processes to ensure compliance with the legislation and continuous performance improvement. It will not be used for any other purpose. We ask for the reference number to investigate any issues that may be highlighted by your responses. Once investigated, all information will be held anonymously and the feedback given will not affect any future requests you may send to the Trust. The results will be evaluated and presented as part of the Information Governance Annual Report.
