Healthwatch Hartlepool is currently looking at patient experience of hospital discharge which has taken place over the last twelve months. We are particularly keen to hear from anyone who has ongoing care needs and lives either in their own home, a sheltered or supported living setting or in a residential care home. Please complete the questionnaire return it to us no later Friday 23rd February.
