Extended Access Service Patient Survey - A34PCN

From Autumn 2022, practices in the A34 Primary Care Network (PCN) will be offering more appointment to patients during the evening and on weekends.  The practices in the A34 PCN are: The Downland Practice (inc. Compton Surgery), Eastfield House Surgery and Strawberry Hill Medical Centre.
This service is called “Extended Access” and it means that additional appointments will be made available to patients across the PCN.
Appointments will be available based on the need of the patient population and to help us understand your needs we are asking if you could spare a few minutes of your time to answer some questions.  Please could you think about when you and your family would need these, perhaps between 7:30am and 8:00am or 6:30pm and 8:00pm on weekdays or between 9:00am and 5:00pm on a Saturday.  The appointments offered will be a mix of telephone and face-to-face with a range of healthcare professionals.  You will be able to book these appointments through your own GP Practice.
Thank you in advance for your response, your feedback will assist greatly in shaping this service going forward.  All survey responses will be confidential.  To help protect your confidentiality, the survey will not contain information that will personally identify you.  Replies are requested by 30th June 2022
