This questionnaire lists all of the information we need to know before we can set you up for submitting live CDS declarations.

Completing the questionnaire does not mean you have to start sending declarations to CDS straight away, but getting all the information and returning it to us as soon as possible will help to get everything configured for when you do want to start.

You must contact each CSP (CNS, MCP, Pentant) with whom you have badges, to register for CDS before you complete this questionnaire. If you don't then you will not be able to complete the questionnaire as the information they give to you (username, password etc.) is required. We also need a list of all the badges you use in Sequoia, as we need to configure these as well.
