Please complete this this short form in order to register for the ALD's Combined insurance scheme for Professional and Affiliate members. Please note that you will continue to be part of this scheme until to write to the ALD Office and ask to be removed.  It will automatically renew each April when we will automatically add the extra payment to your subscription rate.
