How it works
This questionnaire is made up of a set of questions relating to the care and support you have received from Bolton Hospice. Some of the questions have a choice of answer and some give a free text box where you can provide some more detail. It should take about 5-10 minutes to complete.
The information you give us will remain anonymous but will help us to improve our services. No one can identify you from your answers, unless you leave contact details for someone to contact you should you wish.
