We are seeking to improve the Primary Secondary Care Interface across Cheshire and Merseyside.

This commonly relates to relationships, clinical governance, culture, communication and clinical pathways between hospitals and general practice.

Cheshire and Merseyside ICB has a consensus agreement describing expected behaviours which can be accessed here.

We are seeking to both establish a baseline of how the interface is working and also help identify what the key areas of focus should be
